All information on form must be filled out or there will be significant delays in processing.
Southwest Youth Athletic Association, Inc. reserves the right to assess additional fees to individuals for any bank charges incurred or collection efforts pertaining to checks that do not clear the bank. As well, if these fees are not promptly reimbursed to SYAA early registrants will have to pay full registration or an incremental $15.00. The player will not be allowed to be on an active roster if these payments have not been submitted to SYAA before the draft and finalization of the team roster. Non-payment of these fees will result in the submission of the individual to a collection agency.
A sample of collection fees may be as follows:
